Insurance/Billing

Billing/Insurance

The Hruska Clinic currently contracts with Bluecross/Blueshield, Midlands Choice, Medica, and Medicare. If you have different insurance, contact our front desk to see what our options are.   You will be responsible for any deductibles, co-pays, co-insurance and any services not covered by your plan. We strongly encourage you to check with your insurer regarding your specific physical therapy benefits prior to your initial appointment. Should your plan include a co-pay for physical therapy, we ask that it be paid at each visit.  There is also a $15 charge applied at your initial visit to cover any supplies you may be given during your course of treatment as well as administrative costs associated with insurance/billing etc.

Some insurance plans require a prescription/referral for physical therapy from your primary doctor; contact your insurance company before your initial evaluation to see if this is a requirement of your plan. Please contact your physician before your first physical therapy appointment to help ensure receiving full insurance benefits for your services with us.

If you are covered by Medicare, you will need a prescription for physical therapy from your medical doctor. A referral from a different health professional, such as a dentist or chiropractor, will not be accepted by Medicare. Once you have received a prescription, it will expire for use in thirty days. Therefore, please schedule your first appointment within that thirty day time period.

Self-Pay/Out of Network

lf you are Out-of-Network, or want to pay at the date of service because of a high-deductible plan or other reasons don’t worry.  We do our best to maximize the value of your healthcare dollar whether you have insurance or not.   This is done through our typical plan of care that encourages less frequent visits, with one on one care with your PT (not an aide or assistant) and more emphasis on your participation with a home based program.

For our out-of network or self pay patients we discount our hourly rate (similarly to what in insurance company will do for their in-network providers) and provide you with the needed paperwork so you can submit the claim on your own if you desire.  This will allow you to get a discount from us and also get whatever out-of network benefit you are due.  We will, however, require payment at the time of service for each visit.  A typical 1 hour evaluation or treatment will be discounted to $170/hr which is about a 25% discount from normal rates.  If you have further questions on our out-of-network policy, and exactly what that might cost in your specific case do not hesitate to contact us at 402-467-4545.

Workers Compensation/Motor Vehicle Accident

If you have a workers compensation claim, your workers compensation carrier will be billed for your physical therapy charges. In the event these claims are denied, you will become financially responsible for all charges. In the event that you seek legal representation in the settlement of your claim, we ask that you adhere to the policies listed under “Personal Liability/Litigation”.

FAQ

What are your hours of operation?

The Hruska Clinic is open from 8:00 a.m. to 6:00 p.m. Monday thru Thursday, and 8:00 a.m. to 12:00 p.m. Fridays. Our appointment times are scheduled on the hour, with the last appointment time of the day is 5:00 p.m. We are not open on the weekends.

When should I arrive for my initial appointment?

Please arrive fifteen minutes prior to your first appointment for completion of admission forms. If you prefer to complete the admission forms (Click button to the left) in advance, it is only necessary to arrive a few minutes early.

How long will the appointment last?

In general, you can plan each session to last approximately one hour.  The first appointment can sometimes take slightly longer. Patients traveling to the Hruska Clinic from longer distances away may be scheduled for more time.  The secretarial staff will let you know how long your appointment is scheduled for. Generally, follow-up appointments are also an hour long. Check with your therapist to determine your individual needs for treatment time.

What should I bring with me?

Upon check-in for your first session, please provide all information necessary for filing your claims, such as a current health insurance card, doctor’s prescription, workers comp claim number and contact information, auto insurance card and/or accident information, attorney information, etc.  If you have filled out our paperwork ahead of time make sure to bring it with you.  If not, we will have you fill it ou prior to your initial evaluation.  Comfortable clothing is recommended including shorts & your favorite tennis shoes (no sandals or flip flops please). X-rays or MRI reports are appreciated if available. Most of all bring a good attitude and willingness to work to get better!

Who should come with me?

We strongly encourage a family member or friend to accompany patients to their first appointment.  Having a teammate on your side to help you with your recovery is a very valuable tool.  If they know what you are going through and how to help you, you have one more therapist on our team! Young adults or children under the age of nineteen will need permission of a guardian.  Children under the age of 12 will need to have a parent stay with them for each session.

What is the cancellation policy?

NO-SHOW Policy

We understand that sometimes you may need to cancel or reschedule your appointment and that emergencies happen.  To ensure that each patient is given the proper amount of time alloted for their visit and to provide the highest quality of care, it is important for each scheduled patient to attend their visit on time. If you are unable to keep a scheduled appointment, please provide notification as soon as possible so that other patients who are waiting to receive our services may be accommodated.

The following situations are considered a “no-Show” appointment:

1. You do not arrive to the appointment and haven’t contacted us to let us know

2. You cancel the appointment with less than 24 hours notice

3. Late Arrival >30 minutes after your scheduled appointment we will need to cancel the appointment and reschedule

*After the first “No-Show” appointment you will recieve notification and a reminder of this policy.

*If you have 2 “no-Show” appointments, you will be subject to a $25.00 fee.

*All no-show payments must be paid prior to your next scheduled visit.

All situations are unique and communication with our staff is desired to ensure everyone has the best experience and outcome they deserve!

Where should I stay if I come to the Hruska Clinic from out of town?

StayBridge Suites (1501 N 86th St; 402-484-6000)

While it’s not the closest hotel to the Hruska Clinic campus, StayBridge offers complimentary shuttle service to any of your destinations while here in Lincoln, as well as complimentary breakfast and dinner (with happy hour).  They offer a special Hruska Clinic/PRIME rate for our patients as well

New Victorian Inn (225 N 50th St; 402-464-4400)

Within a short walk to the Hruska Clinic and lots of eating choices it is a great central location.  They offer a reduced rate for our patients.  Mention PRIME or Hruska Clinic when you book your room.

Chase Suites (200 S 68th St Pl; 402-483-4900)

A short drive from the Hruska Clinic, Chase Suites offers full suites with full kitchenettes.  Lots of room if you plan on cooking all your meals and need a full suite.

Comfort Inn (331 N Cotner Blvd.)

The Comfort Inn has typically been a great place to stay for our out of town patients as it is within a short walk to our campus.  New management has not agreed to supply us a reduced rate for our patients, however the location is hard to beat.
